Mobile Health Services
SARHA's Mobile Health Center exists to provide easier access and convenience to healthcare needs. From well-visits and screenings to minor injuries and sick visits, SARHA's Mobile Health Center team is ready to assist our communities and schools by BRINGING QUALITY HEALTH CARE TO YOU!
If you are the Parent or Guardian of a student at one of the schools we are partnered with, the required paperwork for your child to be seen at any time on our mobile unit can be printed below. Please print and fill it out completely and return it to your child's school. You can also receive a copy of the required paperwork from your child's school nurse.

Southeast Alabama Rural Health Associates (SARHA) is committed to meeting the needs of all patients and provides in-house translation services at various locations, as well as telephone translation services during and after hours at all SARHA locations.
